Instructions to complete your application for admission:
1. You must fill out the application in full in block letters using a black or blue pen.
2. Your application must be accompanied by the admission fee of $75.00. Include your payment by check or money order payable to the School of Nurses / Anesthetists of Puerto Rico.
3. Submit your request personally or send it to the following postal address:

4. Ask the academic institutions in which you have studied to send an official copy of your credit transcript indicating your degree/s conferred (s) (to apply) to the postal address indicated above.
5. Request the two (2) letters of recommendation. These should be addressed directly to the postal address indicated above.
6. Request evidence of hospital work experience at High Risk. These should be addressed directly to the postal address indicated above.
7. All documents submitted in the eligibility process for admission to The Puerto Rico School of Nurses Anesthetists (PRSNA) become the property of the Institution and will NOT be returned.
8. Only applications completed in full will be considered to continue the process of admissions to the Institution.
9. People with disabilities, who do not allow them to access our offices located on the second floor, can contact the Office of Administrative and Student Affairs at 787-998-8997 to coordinate services in one of our offices with access for people with impediments

*** If you choose to fill out the online application, you must send the admission fee payment by mail to begin the application process.
You can fill out the application online as long as you meet all the requirements and can send the required documents by regular mail.

1. Deberá llenar la solicitud en su totalidad en letra de molde utilizando bolígrafo negro o azul.
2. Su solicitud tiene que ser acompañada por la cuota de admisión de $75.00. Incluya su pago en cheque o giro postal pagadero a la Escuela de Enfermeras/os Anestesistas de Puerto Rico.

4. Solicite a las instituciones académicas en la cuales ha estudiado que envíe una copia oficial de su trascripción de créditos que indiquen su grado/s conferido/s (que apliquen) a la dirección postal arriba indicada.
5. Solicite las dos (2) cartas de recomendación. Estas deben ser dirigidas directamente a la dirección postal arriba indicada.
6. Solicite la evidencia de experiencia laboral hospitalaria en Alto Riesgo. Estas deben ser dirigidas directamente a la dirección postal arriba indicada.
7. Sólo solicitudes completadas en su totalidad, serán consideradas para continuar con el proceso de admisiones a la Institución.
8. Personas con algún impedimento, que no les permita acceder a nuestras oficinas ubicadas en el segundo piso, pueden comunicarse a la oficina de Asuntos Administrativos y Estudiantiles al 787-998-8997 para coordinar los servicios en una de nuestras oficinas con acceso para personas con impedimentos.
*** Si selecciona llenar la solicitud “online” debe enviar el pago de cuota de admisión por correo para comenzar el proceso de solicitud. Puede llenar la solicitud en línea siempre y cuando cumpla con todos los requisitos y pueda enviar los documentos requeridos por correo regular.
